How can this program help me?
If you qualify for the program, PatientOne will provide free oncology medicine to you shipped to your physician. Approved product requests are conveniently shipped to your doctor’s office.

Do I qualify for PatientOne?
For your physician to receive free oncology medicine, you must:
  • Have no medical insurance or your insurance does not cover therapy.
  • Be prescribed a qualified Lilly oncology drug that will be administered in the US.
  • Be in ongoing therapy.
  • Be a United States resident.*
  • Have submitted an application to the PatientOne program within 90 days of treatment
  • Meet the household income guidelines for the program.

Where would I get my medicine?
Upon receiving an approved product request from your physician, replacement oncology products are shipped to your doctor's office.

How do I apply?
  • Contact your healthcare provider to inquire about filling out an application
  • An application is available at LillyPatientOne.com or by calling 1-866-472-8663.
  • Note: You must provide your healthcare provider with proof of income.

Am I eligible for the program while I am pending other coverage?
The PatientOne program may provide product assistance to qualified applicants for up to three months pending other coverage such as Medicaid, Medicare, or Department of Veterans Affairs (VA) healthcare coverage.

Keep a photocopy of your application and your original income documents. If approved, your healthcare provider will be notified by the program and asked to complete a Dosage Tracking form for approval. Upon approval, the medication will be shipped to your healthcare provider’s office. You will remain enrolled in the program for up to 1 year. To continue receiving free Lilly medicines, your healthcare provider must reapply each year.
